undersigned desiring to enter and participate in the Rollin for Kids Car and
Motorcycle Show, June 12, 2005 at the
Fender Center for the Performing Arts facility located at 365 N. Main Street,
Corona CA 92880; do hereby tender his/her
application. In consideration acceptance of this application, the undersigned
does hereby; 1) Release the City of Corona,
Fender Center, Rollin For Kids Volunteers, Corona Rotary Club, their respective
staff, directors, volunteers and agents, from
any and all liability arising out of said event; 2) Release all other participating
business facilites and participants from any
and all liability arising out of said event; 3) Give permission to use any
photographs or information obtained prior to and at
the event for publication, advertising and video filming of the event; 4)
Agree to have vehicle liability insurance as required by
the State of California DMV - CVC 16020 section (a-d). I have read the above
and accept the terms and conditions.
